Through medical assessment on functional disabilities caused by cardiac disease, our multi-disciplinary team will provide rehabilitation training program which focuses on training of cardio-pulmonary function and muscular exercise for individual patient. The team will also assist patients to make positive change on their lifestyles, improve quality of life and reduce the opportunity of recurrence and mortality. - Target Patients:
Patients in need of cardiac rehabilitative care, for example, those diagnosed with coronary heart disease, heart failure, or have recently had heart attack or cardiac surgery.

- Cardiologist ( 4 sessions )
- Individual Dietitian Consultations ( 4 sessions )
- Individual Physical Training Sessions ( 11 sessions )
- Individual Educator Consultations (3 sessions )
- Health Assessments ( Pre- and Post- Program )
